Appraisal Management Companies

The following are required forms for changing a Control Person or Main Control Person (reporting required within 30 days of ownership change):

• AMC Notification of Change Form
• New Control Person Application* , Main Control Person Application** , Minor Owner*** (for each addition)
• Two fingerprint cards (blue FD-258) (for each addition)
• Letter of Waiver - Consent to Background Check Form (for each addition)
• $40 fingerprint/background check processing fee (for each addition)

*Control Persons are all persons directly or indirectly owning 10% or more of an AMC

**Main Control Person is the designated responsible person directly or indirectly owning 10% or more of an AMC

***Any and all owners with an ownership interest of 10% or less
